IDIOPATHIC GRANULOMATOUS MASTITIS: A DIAGNOSTIC DILEMMA, OFTEN A MISDIAGNOSED ENTITY

Abstract

Background & objective: Idiopathic granulomatous mastitisof breast is an uncommon condition with unknown aetiology. Incidence of granulomatous mastitis is 2.4 per 10,000.Clinical& radiological findings may often mimic abscess, carcinoma of thebreast and hencemultimodal approach is necessary in diagnosing and adequately treating the condition. Methods: We conducted an observational study in Dr.D.Y.Patil medical college hospital & research centre. All thehistologically proven idiopathic granulomatous mastitis patients werestudied during a study period of 2 years. Informed consents were taken from all the patients in the study. We studied the clinical features, radiologicalfindings,histopathological reports and assessed their response to various modalities of treatment. Results: Histologically proven cases of idiopathic granulomatous mastitis were studied. We had 14 patients, out of which 6 patients (42.8%) had lump as the predominant symptom. 5 patients (35.7%) presentedas breast abscess. 2 patients (14.2%) presented with mastalgiaand 1 patient (7.14%) presented as fistula. Patients presenting as an abscess underwent Incision and drainage. The patients that underwent incision and drainage presented with recurrence and had to undergo repeated incision and drainage followed by biopsy. Biopsy report showed granuloma formation and hence were started on corticosteroids. Response to corticosteroid therapy was assessed. 6 patients were posted for lumpectomy without corticosteroid therapy and recurrence was noticed in 4 patients (66%) Recurrent granulomatous mastitis patients were subjected to lumpectomy with long term corticosteroid therapy. Conclusion: Surgery along with corticosteroid therapy showed good result compared to surgery without corticosteroid therapy.
