EVALUATION OF SINO-NASAL DISEASES BY MDCT AND TO CORRELATE WITH HISTO-PATHOLOGICAL FINDINGS

Abstract

Background and aim There is a wide range of disorders that affect the paranasal sinuses, from infections, inflammation to benign or malignant neoplasms. Sinuses are closely related to the orbit, cranial fossa & pterygopalatine fossa anatomically. Early involvement in these areas is therefore essential. Diagnostic radiography is crucial because the surrounding bony structures make clinical assessment difficult. The goal of the current study is to correlate histological findings with the appropriate diagnosis of PNS illnesses. Methods: For 93 patients, a prospective cross sectional hospital based study with clinically suspected PNS disorders who were referred from the departments of otorhinolaryngology and head and neck surgery, both axial and coronal CT scan studies (done with GE, spiral CT machine) were performed. Results: CT diagnosis had shown similar sensitivity, specificity, PPV and NPV in diagnosing various sinonasal diseases when correlated with HPR. For fungal sinusitis the sensitivity was 100% and specificity was 86.2%. Polyps have sensitivity of 100% and specificity of 97.01%. For Angiofibromas the sensitivity was 100% and specificity was 98.81%. For mucoceles the sensitivity was 100% and specificity was 98.82%. Other neoplasms have sensitivity and specificity of 100%. The p-value in all instances was 0.05, i.e. 0.0001 indicating the significance of the findings. Conclusions: In order to assess various congenital, inflammatory, benign, and malignant diseases and associated consequences and plan the patient's future care, CT is the imaging technique of choice for the sinonasal region. However, it may be difficult to distinguish between fungal sinusitis and thick secretions.
