CORELATION OF GLEASON GRADING WITH SERUM PSA LEVELS IN PROSTATIC ADENOCARCINOMA

Abstract
Background - Adenocarcinoma is most common pattern as it develops primarily from the glandular part of the organ and shows typical glandular patterns on microscopic examination. The most common grading system for prostatic adenocarcinoma is gleason grading system. Increased PSA levels are seen in all prostatic diseases but markedly elevated levels are indicative of carcinoma prostate. Present study was done to evaluation of serum PSA levels correlation with gleason grading system. Methods – There were total of 58 confirmed cases of prostatic adenocarcinoma. Paraffin-embedded hematoxylin and eosin stain sections were studied and gleason grading was assigned by examining pathologist. Gleason grading was assigned in 5 gleason grading group. The PSA levels were estimated using automated Chemiluminescence method on Beckman Coulter Access –II System. Results - Out of 21 cases which had PSA level 4-10, most no of cases were in gleason group 1 (10 out of 21) and gleason group 2 was second most common for this value (06 of 21). While if we see results in markedly raised PSA levels (> 100) there were 6 cases – 3 were assigned gleason grade 5 reflecting poorly differentiation with one and two cases in group 3 and group 4 respectively. Conclusion -PSA level and Gleason grading or tumor differentiation have positive corelation. Higher PSA level more are chances of poorly differentiated Prostatic adenocarcinoma.
